This Application Form is part of the application process. Completing this application does not guarantee acceptance. Once your form is received, you will be contacted to arrange an appointment with the school administration. Please do not phone the school to verify your submission. At the time of registration with our Administrator, we will ask to view the following documents: • Birth certificate • Baptismal certificate • and if applicable: Permanent Residency Card / Passport / Citizenship Documents We thank you for your interest in Catholic Education, and look forward to meeting with you.
Visual confirmation of your child's Birth Certificate , Saskatchwan Health Card are required when registering. We will also need to see your child's Catholic Baptismal Certificate and Residency papers if they are applicable. If your child does not have a birth certificate but they were born in Saskatchewan, an application can be made by contacting eHealth. ★ If your child was born outside of Saskatchewan, check Vital Statistics for the province they were born in.
